The following forms must be filled out in its entirety and submitted to the athletic department prior to a student-athlete being cleared to participate in a sport.

All first-time student-athletes at SUNY Canton who are interested in playing a sport during the 2012-11 academic school year must have a physical prior to trying out for a sport. If you are a first-time student-athlete please click on the link below for the Health History and Physical Exam form that must be completed and submitted to the SUNY Canton Health Center.
